New Student Registration

  • Visit the Davis School District Home page and select the school from the drop-down menu at the top of the page. During the school’s office hours listed on their website, arrive in person and provide the following required information: 
    Parent picture identification

    o  Original birth certificate for the student
    o  Original student immunization schedule
    o  Proof of address with a current rent/lease or purchase agreement AND one of the the following: bank statement, utility bill, government letter, postal change of address. 

  • Immunizations required by the state of Utah must be completed and documentation brought to the school for a student to be registered. See the Utah Department of Health Immunization Program for a list of immunization requirements.
