Bio

Suzi was previously a high school principal and Federal Programs Director before becoming the Human Resources Director.  She has a Bachelor’s Degree from Weber State University (English major, Business minor), Master’s Degree from Utah State University, and an Administrative license from Utah State University.  She received the 5A Utah Coach of the Year Award and the National Federation of Interscholastic Sports (NFSIA) Section 7 National Coach of the Year when she taught at Northridge High School.  Suzi was an elementary intern at Adelaide Elementary, assistant principal at Viewmont and Clearfield High School, and the principal at Clearfield High School.  She served as the President of the Utah Association of Secondary School Principals (UASSP) then became the State Coordinator for several years.  She also served on the National Association of Secondary School Principals (NASSP) Board of Directors representing Alaska, California, Hawaii, Idaho, Montana, Nevada, Oregon, Utah, and Washington for four years as the Region 7 Diversity Director and Audit Committee Chair.

Bio

Steve has been with the district’s Human Resources Department since 1998 and currently serves as one of the department’s Associate Directors.  Prior to joining Davis School District, he worked for the Utah Labor Commission and the University of Utah’s Office of Equal Opportunity.  He holds a Master’s Degree in Human Resource Management from the University of Utah, and a J.D. from Brigham Young University.

Bio

Rodney joined the Human Resources Department in 2007.  His previous positions with Davis School District includes five years as the HR Manager at PARC and four years as a teacher in alternative education.  Rodney worked as the HR Manager for Barnes Aerospace prior to his employment with the District in 1998.  He earned a bachelor’s degree in English from Weber State University, a master’s degree in human resources from Utah State University, and a Ph.D. in educational leadership from the University of Utah.

Payment of Fees

New Employee Training

  1. Go to the district homepage at www.davis.k12.ut.us
  2. Click on the "Employees" tab
  3. Click on "DSD Canvas"
  4. Click on "Canvas Login"
  5. Enter your district email for the username.  Your password is your first name with the first initial capitalized followed by your employee ID number - no spaces or dashes.  If your password is less than 8 characters, you will need to add 0s to the end until 8 characters are reached.
  6. Hover your mouse over "Courses"
  7. Select your course from the dropdown menu that appears
  8. Click on "Modules"
  9. Start from the top of the list and click on a lesson to view
  10. Click on the video lesson to play
  11. Click to enlarge the lesson to full screen size if desired.  Click again to minimize lesson size.
  12. Click on "Next" to go to the next lesson segment or quiz.
  13. After completion of each Module's quiz, click on "Grades" to see score received.

Whether you are a current educator due for license renewal or an inactive educator looking to renew an expired license, you can find the most current instructions from the Utah State Board of Education's website here: USBE renewal site 

CACTUS

CACTUS is the database for USBE containing Utah educator licensing information.  You can log in to your CACTUS file here: CACTUS login

If you've forgotten your UEN login credentials, please call one of the phone numbers listed at the bottom of the CACTUS login screen.  The district does not have access to your UEN login credentials.

MIDAS

MIDAS is the system built by USBE for collecting and tracking professional learning activities for educators.  You can find information about MIDAS and a link for logging in here: MIDAS login

Routes to Becoming a Teacher

Praxis PLT

A Praxis PLT passing score is required for secondary, elementary and special ed (K-12) license holders only.  Scores take 4-6 weeks to post on your CACTUS record, so take the test before May!

Upgrading to a Level 2 License

How do I upgrade my level 1 license to a level 2 license?

New Teacher Induction Program

Complete your new teacher induction program requirements during the period of your level 1 license.  Program requirements are based on the area of license that you hold, not your assignment.  For more information, contact:

Suicide Prevention Training

  • BOTH HOURS must be completed by all educators within the current license cycle and documented
  • Online Hour: watch the video, complete the quiz and save the certificate
  • Classroom Hour: attend a district sponsored training and save the certificate
    • Receive training sponsored by your school administration OR
    • Receive training sponsored by the district through the Student Services Department OR
    • Attend a District QRP Training by contacting the K-12 Prevention Coordinator, Christi Blankman

Ethics Review

A USBE ethics review must be completed within 12 calendar months of the exact date that you finalize your upgrade electronically.  Click the button below to visit USBE's Educator Licensing Online webiste and click on the Educator Ethics Review button.

Background Check

  • If you have not completed a background check for USBE since July 1, 2015, you need to do another one.
  • After you have applied for the background check, USBE will send you an email with your fingerprint authorization form (this may take anywhere from 15 minutes to 24 hours).  You must bring this form with you to a livescan site in order to be fingerprinted.
  • If you get an error message telling you that you do not need one, this may be incorrect if you are upgrading out of cycle. Please contact Jocelyn Heyne in Human Resources before you skip this vital step! email: jheyne@dsdmail.net

License Upgrade Form

  • AFTER your final summative evaluation has been completed, meet with your supervisor and begin the license upgrade form.
  • Provide your supervisor with proof of BOTH hours of suicide prevention training
  • The EYE form must be submitted to Jocelyn Heyne in Human Resources by either yourself or your principal for completion. It is YOUR responsibility to make sure the form as been sent to HR.  If you choose to use the electronic format, then email the form directly to Jocelyn at jheyne@dsdmail.net The deadline for receiving forms is June 20th.
  • You will receive an email from Jocelyn Heyne in Human Resources with instructions for finalizing the upgrade electronically once you have met all requirements.
  • Online upgrade must be completed no later than June 25th.

This form is used when requesting an exclusion of a substitute teacher. It may be completed by the classroom teacher or site secretary but must be reviewed, signed, and submitted by a School Administrator.

Job Review Form (For Classified Employees)

Long Term Substitute Pay Request Form

This form is used by school administrators to request long-term substitute pay. The selected substitute teacher will receive long-term pay regardless of their qualification as long as the assignment reaches 20 consecutive days or more. Ensure the form is signed by an administrator and send the form to Anjel in the SubSystems Office.
