Call the office for an appointment at Case Management.
This meeting is held on Tuesday afternoons. The appointments are usually about 10-15 minutes. Case manager's must be present for placement. |

You have two choices:
CHOICE #1 You may print the Youth in Custody form (PDF form) from the link above. Fill out a form by hand, completely, for each student that has an appointment. Fax the completed forms to 801-402-0474.
CHOICE #2 The link below in Step 3 is a Microsoft Excel form. DO NOT open and fill out the form from this website. SAVE the form to your desktop. Open with the Excel program. Use your TAB key to navigate to each blank that needs to be completed. Complete this page and Save it. Attach your file to an e-mail to: celjohnson@dsdmail.net
or... You may complete the form, print the document and fax it to 801-402-0474. Please be sure to include all contact information, your e-mail address, your student's list of medication, and if they will be getting counseling, that counselors name and phone number.

| A Note regarding Special Education Classifications: |
|
Students that are classified "Special Ed" (this could be for learning disabilities, behavior or physical/mental disabilities) will need a current IEP.
Students cannot be placed until the IEP has been reviewed by the Davis District Special Education department. If the IEP is present the process will expidite matters.

| A Note regarding grades and credits: |

If it can possibly be arranged, moving secondary (7-12 grade) students at the term or semester breaks would be the most and highly preferrable times for the move. This consideration would help students keep their credits and avoid the loss of credit and time. If a student must move during the term please make sure they have withdrawn from their current school, turned in all their books and have received a copy of their "Withdrawing" grades to carry to their next school.
PLEASE!! Check calendar in January and February for UBSCT Tests. It is important that any appointments, court dates, therapies, groups, etc. are made for other days so that your student is able to complete the UBSCT tests. Thanks! |
