Davis CARE Award
Employee Recognition Program

   
 
Introduction


   
  The Davis CARE Award is designed to Celebrate Achievement, Reward Excellence. It recognizes and honors exceptional employees in the Davis School District who support and encourage excellence in education.

The award will be given to up to 10 district employees and one Rookie of the Year (first-year teacher) at a dinner in their honor at 6 p.m., May 8, in the Davis Conference Center. Courtesy of Chevron and the Davis Education Foundation, each will receive a $1,000 award.

   
 
Nomination Instructions


   
  Community residents and Davis School District employees are invited to nominate individuals who have demonstrated outstanding contributions toward education. Only district employees are eligible for the award. Prior CARE winners cannot be nominated.

Nominees should fit ALL of the following criteria:

• Achievement in the Field of Endeavor — The nominee demonstrates achievement and excellence in their field of work. These achievements have produced superior results and outcomes in education.

• Innovation or Creativity — The nominee demonstrates a unique approach in the methods, techniques or processes used in their employment. The nominee’s technique or approach has led to superior results.

• Contribution to Improving and Sustaining Quality Education — The nominee’s work has enhanced the quality of the educational experience in the Davis School District.

All applications should be submitted on this approved nomination form, which can be filled out by hitting NEXT at the bottom of this screen. You may hit cancel at anytime if you are not prepared to complete the nomination. A sample form is available online to help you prepare your nomination. Nominations cannot be edited once they are submitted. Handwritten forms will NOT be considered. Self-nominations and those made by immediate family members will not be accepted .

Schools, departments and parent groups are asked to jointly submit a single nomination, rather than multiple nominations for one person. Multiple nominations for one person will be given the same consideration as a single nomination.

Nominations SHOULD include no more than two additional, single-sided, pages of supplementary materials. Supplementary items can include comments or letters of support from individuals directly influenced by the nominee, letter of recommendation from a supervisor or specific examples in support of the application. Supplementary materials may be mailed, e-mailed, faxed or delivered in person, but must be received by the 4:30 p.m. deadline on March 10. Mail to CARE Selection Committee, c/o Kayleen Carter, P.O.
