Yearbooks

Yearbooks are lasting memories: make sure you don't miss out!

Order your yearbook by November 15 in order to insure a copy.

Cost: $18

After December 1, the cost is $23.00

Step 1 Read the Registration Information. It will be mailed to your home, and can also be downloaded here.
Step 2

Print and complete all your forms

  • Registration Receipt
    Following instructions provided on the Course Request page (which you'll receive in the mail), fill out and return the receipt. Students applying for a fee waiver should also fill out a completed receipt. Be sure to print a second copy for your records.
  • Student Information Form (will be mailed)
    Make corrections, complete all sections, list contact names in case of emergency and sign.
  • Computer Use Agreement
    Parent/Guardian and Student signature required. All students must have this agreement on file at the school in order to use school computers.
  • Fee Waiver Application
    Apply if qualified for free lunch or Federal/State help. (Individuals with specific financial needs may contact the school principal.)(Not fillable online. Print and complete.)
  • 7th Grade Immunization Information
    Click here for more information about required immunizations. Have your doctor fax a copy of shot records to 402-6101 or pick up a pink immunization card in the school office, take it to your doctor to have it filled out, and return it to the school.
    Required for seventh grade registration.
Step 3 Send all completed information, along with your check or money order, to the CJH Office by July 20th. Make your check payable to Centerville Jr. High.
