The two steps are: 1) have your doctor sign the physicians form (found under forms on this site). 2) go online and complete the Personal Health Assessment (PHA) link: http://www.h2u.com/mountainstar. This year's access code is MSDSD109. The deadline to get everything completed is May 15, 2017.

What is Davis Moves?
Our employees are our most valuable asset. Our intent in implementing a wellness component to our insurance program is first to provide a means for our employees to become more aware of their own health and health risks. Secondly, we recognize the urgency of bringing health cost in check to enable our employees an opportunity to receive financial rewards for the work that they do.
Any employee who chooses to take advantage of Davis School District health insurance benefits as of September 1 of each year, will be assessed a wellness premium of $6.25 each month for the 10 months starting with the September 30th check. This premium will be a separate line item on their check but is part of the overall insurance premium. The district will collect a total of $62.50, you have the opportunity to get $60 of that money back. There are two simple requirements that you need to complete, 1) biometric screening, by going to one of our health fairs or by going to their doctor 2) Complete an online Personal Risk Assessment (PRA) on the h2u|Workswebsite at http://www.h2u.com/mountainstar
