The Davis Education Foundation in cooperation with Davis School District is refurbishing computers and making them available to students who would not otherwise have access to a computer at home.  We have a limited number of these computers and, therefore, wish to award them to students who have the greatest need and would make the best use of the gift! If you have a kindergarten student that will be attending Ellison Park Elementary during the 2013-2014 school year please come to the office.

 

Please bring the following items to registration:

1. Certified original birth certificate

2. Proof of Residency. (Copy of deed or most recent mortgage payment OR copy of lease agreement, AND one other document showing your residency, i.e. payroll stub, bank statement, utility bill, vehicle registration, etc.)

3. Required Immunizations

4. Guardianship information

5. Guardian Picture ID

6. Activity donation $5.00 (Correct change, we can't make change)
