It is important for the school to have pertinent health information about each student. If your student has a health care concern, please list it on the Bountiful Elementary Student Information Form in the space for HEALTH PROBLEMS. IF THERE ARE NO PROBLEMS, PLEASE WRITE 'NO CONCERN' in this space.

If your student will be in Kindergarten or new to the school or has a major new health concern, please fill out and sign the full page health information form. This will be used to generate a health care plan for your student if needed. Medications kept in the office will require additional paper work to be submitted when the medication is accepted by the school secretaries.
